Engaged Communities. Healthy Communities.

Welcome to your Central East LHIN (Local Health Integration Network) and its first Integrated Health Service Plan (IHSP).

This plan represents the first stage in the transformation of Ontario's health care system, from centralized to local health care system planning and funding. It is an initial blueprint for change, a three-year plan towards building a better health care system. Our ultimate goal is to realize our vision of Engaged Communities. Healthy Communities.

IHSP & Appendices

Creating the IHSP, with significant input from community residents and health care providers, was a complex undertaking compressed into a short-time frame. Executing the priorities for actions identified in the IHSP will be even more complex. Given the challenges of implementing this change management exercise, it is critical that a disciplined project plan be developed.

The IHSP Work Plan identifies at a high-level the themes and sub-themes of activities that will be carried out by the LHIN staff, its planning partners, and health care providers over the next three years. The Draft IHSP Work Plan will be used for the following purposes:

  • Communicate with stakeholders on how the LHIN will execute the IHSP, and their respective roles within that plan;
  • Obtain feedback from the three Networks and nine Collaboratives;
  • Give direction to the priority of activities undertaken by the Networks and eventually the Collaboratives; and
  • Create new planning partnerships (e.g., work groups) and relationships (e.g., public health).

Additional information regarding the implementation of the IHSP can be found behind the Resource Documents button and specifically the Community Engagement and Integration pages.
