Seamless Care for Seniors
VON Canada (Victorian Order of Nurses) has launched a groundbreaking new tool for Canada's nearly three million caregivers: "Caregiver Connect...from Caring to Sharing/De l'entraide au partage...aidants en réseau." This innovative bilingual web portal provides access to information and resources that caregivers need to care for themselves, to provide better quality care to their family members and/or friends, and to connect and share with other caregivers through a virtual discussion forum. The Frail Elderly Alliance of Durham Region has created Durham's first Seniors Programs and Service Website.
Chronic Disease Prevention and Management
The Canadian Diabetes Association has compiled a resource piece for Financial Assistance Programs for individuals with diabetes, many of which are provided through the Ministry of Health and Long-Term Care.

In 2004, the Heart and Stroke Foundation of Ontario launched its High Blood Pressure Strategy that identified one simple goal: To significantly increase the number of Ontarians with hypertension who are optimally managed and controlled. This goal is achieved through:
- Patient Support: Providing enabling tools and support to patients and their families to better self manage hypertension
- Provider Support: Providing tools and support to improve the diagnosis and treatment of hypertension by family health providers.

Mental Health and Addictions
A Durham Region Dual Diagnosis Resource Guide for People with a Dual Diagnosis has been prepared by the Durham Region Dual Diagnosis Committee for consumers, family, friends, advocates and service providers within the Durham Region. The intent of this book is to provide information about services and supports for people living with a dual diagnosis. These include services in both the developmental and mental health sector. Hong Fook Mental Health Association has recently published. "Understanding, Support and Self-care: A Resource Manual For Family Members and Caregivers" which is available in Chinese (simplified & traditional versions), Korean and Vietnamese on Hong Fook's website as well as in hard copies. This resource manual is designed for family members, caregivers and friends of individuals with mental illness who are interested in learning about issues related to mental health and mental illness.
