Carriage House helps people regain health and quality of life
By the time 66-year-old Rose McMullen was admitted to Oshawa Hospital, she was so sick with severe pneumonia that she had lost nearly 40 pounds from her already slight 100-pound frame. Rose, who lives with muscular dystrophy and arthritis, had been mostly housebound for years and did not realize how ill she was. After two weeks of medical treatment, Rose’s health improved to the point where she no longer needed 24-7 acute hospital care. However, after her in-patient stay, the thought of going home made her anxious.

Durham Regional Police, Mental Health Support Team (MHST) bring Chronically Homeless 56 Year Old Man to CMHA Durham’s Walk-In Services
On January 24, 2018, the MHST brought Mr. Gauci to CMHA Durham’s Community Connection Services program due to his chronic homelessness. He had been living on the streets between Ajax and Oshawa since August 2017 and did not know how to access his Ontario Disability Support Program (ODSP) income entitlements or establish a residence for himself. His only possessions were the clothes he wore and the plastic bag he carried with some food. Mr. Gauci established a connection with CMHA staff returning daily to check in with staff, access our teaching kitchen for lunch, emergency food pantry and shower and laundry facilities... read more.

Patient Story: Harry and Bev
Harry and Bev have lived in Peterborough County for almost 30 years. They moved to the area to raise their two daughters and welcomed their first grandchild a few years ago. Harry enjoyed a successful career in the telecommunications field and was a volunteer firefighter for over 20 years.
On November 14, 2008, Harry and a telecommunications colleague were fixing a residential phone line at the side of a road when he got a headache he describes now as the “worst pain you could imagine”. From what he knew from the media on the signs of a stroke and with his experience with the local fire department, Harry guessed he was having a stroke and told his colleague to call 911. He told his colleague his age, doctor’s name and that he wasn’t on any medications. But this would be the last thing he remembered doing for the next three weeks. Harry had suffered a ruptured cerebral aneurysm in the anterior communicating artery.

Frost Manor BSO team working to decrease wandering with new hallway murals
Frost Manor’s Behavioural Supports Ontario (BSO) team is working towards decreasing wandering in residents with cognitive impairment with a new project to help make navigating the Lindsay long-term care home’ s hallways easier. Residents with cognitive impairment are often challenged to find their rooms. Each of the halls at Frost Manor look similar, which can add to the confusion, says registered practical nurse Justin Hills, who recently became Frost Manor’s BSO lead, thanks to funding from the Central East Local Health Integration Network (LHIN).

Hillsdale Estates demonstrates person-centered care in action with inspiring stories of success
The person-centered care movement in long-term care homes is about altering the institutional norms and attitudes of a facility to a more inclusive and engaged human model. It encompasses many different values that seek to improve quality of life for residents and empower staff to respond to various needs in a more personalized way. The Region of Durham Social Services Long Term Care Homes have been working to co-create a person-centered care model across all four of their Long-Term Care homes. By engaging residents, families, and staff at all levels, together they are exploring how person centered-care will look in the homes.

Reducing surgical wait times with access software
Five years ago, the Central East Local Health Integration Network (Central East LHIN) took a bold step to remedy their surgical wait times. Representing one of Ontario’s fastest growing geographic regions, the Central East LHIN needed a solution to enable better management of surgical wait times at the seven hospitals conducting surgery under its jurisdiction and offer support to surgeons needing to manage their wait lists. That solution was Novari Health’s web-based Surgical Access with Smart Wait software system.
