Hip and Knee Rapid Access Clinics (RAC)
A standard referral form for the hip/knee pathway is available for primary care providers (PCP) below. Referrals must be completed by a PCP.
In collaboration with the patient, the referral form is completed by the PCP and sent to MSK Central Intake. On the referral form, the PCP indicates the patient’s preference of:
- Preferred surgeon
- First available assessment*
- First available surgeon*
- Preferred hospital
- Closest to home
*If a ‘first available’ option is selected, the patient will be referred to a RAC located anywhere across the Central East LHIN, depending on wait times (locations below).
Hip and Knee RAC
Once Central Intake receives the referral, it is processed and sent to the RAC based on the patient’s choice. Hip and Knee RACs are located across the Central East LHIN:

At the RAC, patients will be assessed by a regulated healthcare professional (Assessor) who has received specialized training with orthopaedic surgeons. The patient will be provided with education and tools to better support their condition, and may be referred to an orthopaedic surgeon.
Surgical Consultation
Patients who are referred for surgical consult will be offered their choice of first available surgeon, preferred surgeon or preferred hospital. The PCP will also be informed of the recommended course of treatment.
