Central East LHIN 2019-2022 Integrated Health Service Plan (IHSP 5)

At the Central East Local Health Integration Network (Central East LHIN) we plan, fund, integrate and monitor the local health care system, and plan and deliver home and community care services throughout seven sub-regions in Scarborough, the Region of Durham, Northumberland County, Peterborough City and County, and Haliburton County and the City of Kawartha Lakes.

And, we do not do it alone.

We are proud to work closely with highly motivated and engaged patients and caregivers, doctors and nurses, Health Service Provider (HSPs) organizations, Service Provider Organizations (SPOs), community partners (including Social Service agencies, Municipalities, Public Health, Emergency Management Services, and Educational Institutions) and the general public, all equally passionate and dedicated to building a high-quality, innovative, patient-centred local health care system.

It is in support of this work that we are now in the process of developing our next local health system’s strategic plan.

Known as an Integrated Health Service Plan (IHSP), the IHSP provides the foundation for the Central East LHIN to define and outline how it will achieve its mission, vision, values and strategic initiatives and priorities over a three-year period, from 2019 to 2022.

The Central East LHIN has been guiding and leading the local health system to achieve shared goals and deliverables since IHSP 1:

IHSP #1 – 2007-2010

Engaged Communities. Healthy Communities

Improving the health of communities through an integrated health care delivery system focused on wellness, equitable and timely access to care that delivers high quality outcomes.

IHSP #2 – 2010-2013

Save 1 Million Hours – Save 10,000 Days

Supporting hospitals and community organizations to integrate services to reduce emergency department use for all residents, and hospital admissions and length of stays for people with vascular conditions.

IHSP #3 – 2013-2016

Community First

Creating an integrated community-based health system, so that Central East LHIN residents spend more time in their homes and their communities.

IHSP #4 – 2016-2019

Living Healthier at Home

Advancing integrated systems of care to help Central East LHIN residents live healthier at home.

Now as we develop the 2019–2022 IHSP (IHSP 5), we want to connect with our partners, patients, caregivers and residents across the seven sub-regions to hear your thoughts about our local health care system – what’s working and where innovations and improvements can still be made.

With IHSP 5 we are continuing to focus on regional systems of care but we are also taking a population health approach to better identify the health needs within our geographic sub-regions so that together we can continue to improve the organization and delivery of services to better meet those local needs.
