Apr 24, 2018 NEW QUALITY STANDARD ADDRESSES PALLIATIVE CARE FOR ADULTS

Health Quality Ontario and the Ontario Palliative Care Network have released a new quality standard with accompanying patient guide and recommendations for adoption:  

Palliative CareCare for Adults with Progressive, Life-Limiting Illness 

Palliative care is an approach to care that aims to relieve suffering and improve quality of life for people with a progressive, life-limiting illness; and for their families and caregivers. It is not just for end-of-life and can be started as early as the time of diagnosis. 

 

It is care that addresses all aspects of your well-being—physical, psychological, social, spiritual, linguistic, cultural, and practical. 

 

This standard focuses on palliative care for people in all settings, including home and community, hospice, hospital, and long-term care.

Mar 27, 2018 HQO Quality Standards Communications Toolkit: Dementia Care in the Community

NEW QUALITY STANDARD ADDRESSES DEMENTIA CARE FOR PEOPLE LIVING IN THE COMMUNITY 

Health Quality Ontario has released a new quality standard on Dementia: Care for People Living in the Community. This standard was developed because of the 111,000 people in Ontario with dementia who are living in the community, many who want to live independently and to participate in decisions about their care. 

This newest quality standard focuses on primary care, specialist care, hospital outpatient services, home care, caregivers, and community support services.  It complements another quality standard Behavioural Symptoms of Dementia which addresses quality care in hospitals and long-term care homes.

Mar 07, 2018 Health Quality Ontario releases three quality standards on Opioids

HEALTH QUALITY ONTARIO RELEASES THREE NEW QUALITY STANDARDS THAT ADDRESS OPIOID PRESCRIBING, OPIOID USE DISORDER 

 

In recent years, Ontario has seen a dramatic rise in the rate of opioid prescribing along with increases in opioid-related deaths, hospitalizations, and emergency department visits. There has also been a growth in the rates of opioid use disorder. The patterns for these issues varies dramatically across the province showing a need for quality standards in this area.  

 

In response, Health Quality Ontario has developed three quality standards, that outline for clinicians and patients what high-quality care looks like when: 

These quality standards also explain why it is important for prescribers to not suddenly discontinue prescription opioids. Quality standards are developed in collaboration with patients with lived experience, physicians, nurses caregivers and organizations across the province and are based on the best available evidence.

Feb 28, 2018 Report to the Community - Strengthening Connections - Winter 2018

We are now over six months into our transformative journey as the renewed Central East Local Health Integration Network (Central East LHIN). With the passing of the Patients First Act, 2016 and through the seamless transfer of accountability for the delivery of Home and Community Care into the LHIN, we now have the tools, oversight and accountability needed to continue to lead the advancement of an integrated sustainable health care system that ensures better health, better care and better value.  Going forward, our expanded mandate will see us have greater involvement with primary care and improved linkages with population health planning. As a result, LHINs and other partners are strengthened in their ability to improve the accessibility and consistency of patient care through an integrated system of care that is focused on the health of the overall population.

Aging with Confidence: Ontario's Action Plan for Seniors empowers people to make the choices that are right for them when it comes to their care, their independence and how they access government services -- whether that's finding ways to keep up an active lifestyle or getting the support needed to live at home longer. The number of seniors in Ontario is forecast to double to 4.6 million within the next 25 years -- and Ontario aims to become the best place in the world to grow older. Aging with Confidence focuses on areas that seniors say they care about most, including living independently for as long as possible and staying connected through social, recreational and volunteer activities. To ensure Ontario is ready for the changing realities of an aging population, the plan also substantially expands the number of long-term care (LTC) beds across the province, while increasing staffing levels and support for LTC home residents.

MENTAL HEALTH AND ADDICTIONS - PLANNING

The Central East LHIN, in concert with Ontario Shores and Lakeridge Health, engaged Deloitte to review the current state of Mental Health and Addiction (MHA) system, examine best practices, and explore opportunities for tangible system improvement in service delivery. The completed Deloitte report was presented to the Central East LHIN Board of Directors at the June 28, 2017 Board meeting, with the six recommendations contained in the report confirmed as guiding principles for the development of a MHA Strategy fostering transformative change and the development of a regional mental health system.

The purpose of this document is to provide a framework for stakeholders who are interested in the planning, development and implementation of residential hospice in the Central East Local Health Integration Network (Central East LHIN). This document was developed from evidence based documentation including research, recommendations and standards from the Strengthening Ontario's End-of-Life Continuum: Advice Regarding the Role of Residential Hospices, Provincial Residential Hospices Working Group (March 2015) and the Standards for Community Residential Hospices, Hospice Palliative Care Ontario (September 2012).

In June 2015 the Central East Local Health Integration Network (LHIN) approved the Durham Mental Health Services, Canadian Mental Health Association – Haliburton, Kawartha, Pine Ridge (HKPR), and The Scarborough Hospital to continue as Project Leads on the next phase (Phase 2) of the Community Crisis Service Review Project. The crisis response services reviewed were those funded specifically under Multi-Sector Service Accountability Agreement (MSAA) Crisis Intervention Functional Centres. The Phase 2 work focused on the review and further definition of the recommendations made in the first phase, and to develop strategies for implementation, including costs and timeframes, to meet the proposed future state.

The Assertive Community Treatment Team (ACTT) Implementation Phase - Year 1 Progress Report (2014-2015) was submitted to the Central East LHIN on May 29, 2015. The report evaluates the introduction of the Stepped Care Model and provides recommendations on future steps.

An Expression of Interest was released by the Central East LHIN (CE LHIN) in 2013 for health service providers to participate in the Community Crisis Service Review Priority Project. The purpose of the project was to review the current state of the community crisis response system, identify gaps and opportunities, and make recommendations for improvements.

The Sustainable Access initiative described in this report is a health services planning project that assesses current and future access to post-acute care services, including long-term care, supportive housing/assisted living, residential hospice, home care, institutional rehabilitation and complex continuing care.

May 25, 2011 Regional Specialized Geriatric Services in the Central East LHIN

The Regional Specialized Geriatrics Program Development initiative was a health services integration exercise that has provided recommendations for the establishment of a Central East LHIN Regional Specialized Geriatric Program. this project was aimed at determining how best to coordinate current and future Specialized Geriatric Services, Senior Friendly Hospital initiatives and overall coordination with psychogeriatric services in the region and led to the development of the Seniors Care Network.

May 25, 2011 Specialized Geriatric and Psychogeriatric Services in the Central East LHIN - An Environmental Scan

The Psychogeriatric Services Coordination project was a health services integration exercise, led by Ontario Shores Centre for Mental Health Sciences, to provide a better understanding of our current landscape related to psychogeriatric services. Through an engagement of stakeholders, an environmental scan of existing services was completed, followed by exploration of more integrated approaches to psychogeriatric services. 

Mar 27, 2009 Hospital Clinical Services Plan

INTEGRATION - PLANNING

On February 17, 2009, Dr. George Buldo, Chair of the Clinical Services Plan (CSP) Steering Committee, submitted the "Hospital Clinical Services Plan - Detailed Report" to the Board of the Central East Local Health Integration Network. The report contained 28 clinical service delivery model recommendations and details on three medical leadership frameworks that were developed by over 150 stakeholders - physicians, nurses, allied health professionals, administrators.
